Censés protéger l'organisme, ils se retournent contre les follicules sains et les mettent au repos forcé. Les cheveux tombent par plaques, souvent en quelques semaines. La bonne nouvelle dans le drame : le follicule n'est pas détruit. Il est endormi, pas mort. C'est précisément ce qui rend la repousse possible.\n\nCette distinction change tout côté traitement. Et contrairement aux corticoïdes utilisés depuis des décennies contre la pelade, ce n'est pas un coup de massue immunitaire généralisé, mais un blocage plus ciblé.\n\n### Une cible plus sélective\n\nCette sélectivité compte. Les premiers inhibiteurs de JAK testés contre la pelade bloquaient un large éventail d'enzymes, avec des effets sur l'ensemble du corps. Le ritlecitinib, lui, se concentre sur JAK3 et TEC, des acteurs très présents dans les cellules immunitaires mais discrets ailleurs. L'idée : viser juste, limiter les dégâts collatéraux.\n\n## Ce que disent les essais ALLEGRO\n\nLes chiffres viennent du programme ALLEGRO, un vaste essai clinique mené par Pfizer. L'étude de phase 2b/3 a recruté **718** patients âgés de 12 ans et plus, dans 18 pays. Tous présentaient une pelade sévère, avec au moins 50% du cuir chevelu dégarni.\n\nLe critère principal s'appuyait sur un outil appelé score SALT. Il mesure le pourcentage de cuir chevelu sans cheveux, de 0 (chevelure complète) à 100 (calvitie totale). L'objectif fixé : atteindre un score SALT inférieur ou égal à 20, c'est-à-dire au maximum 20% de cuir chevelu encore dégarni. En clair, retrouver 80% de sa chevelure.\n\n### Les résultats à six mois\n\nÀ la semaine 24, **23%** des patients sous ritlecitinib 50 mg franchissaient ce seuil. Dans le groupe placebo : à peine **2%**. Un écart net, suffisant pour valider le médicament.\n\nMais la repousse ne s'arrête pas là. Elle continue avec le temps. À la semaine 48, soit après un an, la proportion de répondeurs grimpait à **43%**. La patience paie : les follicules réveillés mettent des mois à reconstituer une chevelure dense.\n\n### Et chez les adolescents\n\nC'est l'un des grands apports du ritlecitinib. À deux ans, près de **73%** des patients atteignaient un score SALT inférieur ou égal à 20, et environ **66%** un score inférieur ou égal à 10, soit quasiment toute la chevelure récupérée.\n\n## Effets secondaires : ce qu'il faut savoir\n\nAucun médicament n'est sans contrepartie, et prétendre l'inverse serait malhonnête. Soyons clairs sur les chiffres.\n\nDans l'analyse de sécurité regroupant les patients des essais, la fréquence globale des effets indésirables sous ritlecitinib était comparable à celle du placebo. Les plus courants restent bénins : maux de tête, rhinopharyngite (le simple rhume), acné, diarrhée. Rien de dramatique pour la majorité des patients.\n\nLes événements graves sont restés rares. Quelques cas de zona ont été observés, un effet connu de cette famille de médicaments, qui touche au système immunitaire. Pas la petite plaque isolée qui repousse souvent toute seule. On parle ici de formes étendues, résistantes, qui pèsent réellement sur la vie quotidienne.\n\nUn point honnête, et il compte. Le traitement n'est pas une guérison définitive. La pelade est une maladie chronique.
