Automated behaviors (\"from now on when X\", \"each time X\", \"whenever X\", \"before/after X\") require hooks configured in settings.json - the harness executes these, not Claude, so memory/preferences cannot fulfill them. Also use for: permissions (\"allow X\", \"add permission\", \"move permission to\"), env vars (\"set X=Y\"), hook troubleshooting, or any changes to settings.json/settings.local.json files. Examples: \"allow npm commands\", \"add bq permission to global settings\", \"move permission to user settings\", \"set DEBUG=true\", \"when claude stops show X\". Pass --comment to post findings as inline PR comments, or --fix to apply the findings to the working tree after the review.\n- simplify: Review the changed code for reuse, simplification, efficiency, and altitude cleanups, then apply the fixes. Quality only — it does not hunt for bugs; use /code-review for that.\n- fewer-permission-prompts: Scan your transcripts for common read-only Bash and MCP tool calls, then add a prioritized allowlist to project .claude/settings.json to reduce permission prompts.\n- loop: Run a prompt or slash command on a recurring interval (e.g. /loop 5m /foo). Omit the interval to let the model self-pace. - When the user wants to set up a recurring task, poll for status, or run something repeatedly on an interval (e.g. \"check the deploy every 5 minutes\", \"keep running /babysit-prs\"). Do NOT invoke for one-off tasks.\n- schedule: Create, update, list, or run scheduled remote agents (routines) that execute on a cron schedule. - When the user wants to schedule a recurring remote agent, set up automated tasks, create a cron job for Claude Code, or manage their scheduled agents/routines. Also use when the user wants a one-time scheduled run (\"run this once at 3pm\", \"remind me to check X tomorrow\").\n- claude-api: Build, debug, and optimize Claude API / Anthropic SDK apps. Apps built with this skill should include prompt caching. Also handles migrating existing Claude API code between Claude model versions (4.5 → 4.6, 4.6 → 4.7, retired-model replacements).\nTRIGGER when: code imports `anthropic`/`@anthropic-ai/sdk`; user asks for the Claude API, Anthropic SDK, or Managed Agents; user adds/modifies/tunes a Claude feature (caching, thinking, compaction, tool use, batch, files, citations, memory) or model (Opus/Sonnet/Haiku) in a file; questions about prompt caching / cache hit rate in an Anthropic SDK project.\nSKIP: file imports `openai`/other-provider SDK, filename like `*-openai.py`/`*-generic.py`, provider-neutral code, general programming/ML.\n- run: Launch and drive this project's app to see a change working. Use when asked to run, start, or screenshot the app, or to confirm a change works in the real app (not just tests). First looks for a project skill that already covers launching the app; otherwise falls back to built-in patterns per project type (CLI, server, TUI, Electron, browser-driven, library).\n- init\n- review: Review a pull request\n- security-review","skillCount":111,"isInitial":true,"names":["adapt","animate","arrange","audit","bolder","bun-to-next","cinik-blog-brief","cinik-translator","clarify","colorize","critique","delight","design-taste-frontend","distill","extract","feedbackpin","full-output-enforcement","gsap-core","gsap-frameworks","gsap-performance","gsap-plugins","gsap-react","gsap-scrolltrigger","gsap-timeline","gsap-utils","harden","high-end-visual-design","impeccable","industrial-brutalist-ui","minimalist-ui","netlify-cli","nextjs-wordpress-headless","normalize","onboard","optimize","overdrive","paf-site-magic","polish","quieter","redesign-existing-projects","refero-design","seo","seo-audit","seo-backlinks","seo-competitor-pages","seo-content","seo-dataforseo","seo-firecrawl","seo-geo","seo-google","seo-hreflang","seo-image-gen","seo-images","seo-local","seo-maps","seo-page","seo-plan","seo-programmatic","seo-schema","seo-sitemap","seo-technical","shape","stitch-design-taste","typeset","handoff","deep-research","exemple-workflow","vercel:bootstrap","vercel:deploy","vercel:env","vercel:marketplace","vercel:status","vercel:ai-gateway","vercel:ai-sdk","vercel:auth","vercel:bootstrap","vercel:chat-sdk","vercel:deployments-cicd","vercel:env-vars","vercel:knowledge-update","vercel:marketplace","vercel:next-cache-components","vercel:next-forge","vercel:next-upgrade","vercel:nextjs","vercel:react-best-practices","vercel:routing-middleware","vercel:runtime-cache","vercel:shadcn","vercel:turbopack","vercel:vercel-agent","vercel:vercel-cli","vercel:vercel-firewall","vercel:vercel-functions","vercel:vercel-sandbox","vercel:vercel-storage","vercel:verification","vercel:workflow","update-config","keybindings-help","verify","code-review","simplify","fewer-permission-prompts","loop","schedule","claude-api","run","init","review","security-review"]},"type":"attachment","uuid":"13429d72-c326-4514-850f-07cf756b32dc","timestamp":"2026-06-01T10:22:26.306Z","userType":"external","entrypoint":"cli","cwd":"/opt/automator/cinik-writer-transtor/files/articles","sessionId":"e5b0d32c-b7e6-4f5d-830a-2c1bb8398957","version":"2.1.158","gitBranch":"HEAD"}

Mécanisme, résultats des essais ALLEGRO et profils concernés, sans survente.\n4\t\n5\t---\n6\t\n7\t# Ritlecitinib (Litfulo) : le traitement de la pelade qui change la donne\n8\t\n9\tEn **juin 2023**, la **FDA** américaine a approuvé un médicament que presque personne n'attendait sur le terrain de la pelade. Son nom : le **ritlecitinib**, vendu sous la marque **Litfulo**. La première molécule au monde validée **dès 12 ans** contre la pelade sévère. Et dans l'essai pivot, après six mois de traitement, **23%** des patients avaient retrouvé au moins 80% de leur chevelure. Contre moins de **2%** sous placebo.\n10\t\n11\tPour une maladie qui n'avait quasiment aucune option efficace, c'est un vrai tournant. La pelade, ou [alopécie areata](https://emrahcinik.com/fr/alopecia-areata/), touche environ **2% de la population** au cours de la vie. Elle frappe sans prévenir. Des plaques rondes. Parfois la totalité du cuir chevelu. Parfois tous les poils du corps. On vous explique ici ce qu'est vraiment le ritlecitinib, comment il agit, ce que disent les essais cliniques, et pour qui il est fait. Sans survente. Avec les chiffres réels.\n12\t\n13\t## La pelade, ce n'est pas la calvitie ordinaire\n14\t\n15\tPremier point à comprendre, et il est capital. **La pelade n'a rien à voir avec la [calvitie héréditaire](https://emrahcinik.com/fr/calvitie-heredite/) classique.**\n16\t\n17\tL'[alopécie androgénétique](https://emrahcinik.com/fr/alopecie-androgenetique/), celle qui dégarnit les golfes et le sommet du crâne, est causée par une hormone, la **DHT**, qui rétrécit peu à peu les follicules. C'est mécanique, hormonal, lent. La [pelade](https://emrahcinik.com/fr/quest-ce-que-lalopecie-areata/), elle, est une [maladie auto-immune](https://emrahcinik.com/fr/maladie-auto-immune-perte-cheveux/). **Votre propre système immunitaire attaque vos follicules pileux** comme s'ils étaient des intrus.\n18\t\n19\tImaginez des gardes du corps devenus paranoïaques. Censés protéger l'organisme, ils se retournent contre les follicules sains et les mettent au repos forcé. Les cheveux tombent par plaques, souvent en quelques semaines. La bonne nouvelle dans le drame : **le follicule n'est pas détruit. Il est endormi, pas mort.** C'est précisément ce qui rend la repousse possible.\n20\t\n21\tCette distinction change tout côté traitement. Bloquer la DHT n'a **aucun effet** sur la pelade. Ici, il faut **calmer le système immunitaire**. Et c'est là qu'entrent en scène les inhibiteurs de JAK.\n22\t\n23\t### Quand l'immunité attaque les cheveux\n24\t\n25\tDans la pelade, des cellules immunitaires appelées **lymphocytes T** s'agglutinent autour du bulbe du cheveu. Elles libèrent des signaux inflammatoires, notamment l'**interféron gamma** et une molécule nommée **interleukine 15**. Ces signaux empruntent une voie de communication interne à la cellule : la **voie JAK-STAT**.\n26\t\n27\tPensez à un système de messagerie. Le signal inflammatoire frappe à la porte de la cellule, et les enzymes **JAK** transmettent le message à l'intérieur. Résultat : l'attaque se renforce, le follicule stoppe sa croissance. **Couper cette messagerie, c'est couper l'ordre d'attaque.**\n28\t\n29\tLa pelade peut prendre des formes plus larges. L'[alopécie universelle](https://emrahcinik.com/fr/alopecia-universalis/) fait tomber l'ensemble des poils du corps. L'[alopecia barbae](https://emrahcinik.com/fr/alopecia-barbae/) cible la barbe. Toutes partagent le même mécanisme auto-immun de fond.\n30\t\n31\t## Ritlecitinib : comment ça marche exactement\n32\t\n33\tLe ritlecitinib est un **inhibiteur de kinases**. En clair, il bloque les enzymes qui transmettent les signaux d'attaque. Mais il a une particularité qui le distingue des autres traitements de sa famille.\n34\t\n35\tIl vise **deux cibles à la fois**. D'abord **JAK3**, une enzyme de la voie JAK-STAT. Ensuite la **famille TEC**, un autre groupe d'enzymes impliquées dans l'activation des lymphocytes T. En verrouillant ces deux portes, le ritlecitinib réduit la fabrication des cytokines inflammatoires et calme directement les cellules immunitaires qui agressent le follicule.\n36\t\n37\tAutrement dit, il agit **sur deux fronts du même conflit**. Il intercepte le message inflammatoire et il désarme une partie des soldats. Une fois l'attaque levée, les follicules endormis peuvent reprendre leur [cycle de croissance](https://emrahcinik.com/fr/cycle-du-cheveu/) normal.\n38\t\n39\tLe traitement se prend par **voie orale, un comprimé par jour**. La dose approuvée est de **50 mg**. Pas d'injection, pas d'application locale. Et contrairement aux corticoïdes utilisés depuis des décennies contre la pelade, ce n'est pas un coup de massue immunitaire généralisé, mais **un blocage plus ciblé**.\n40\t\n41\t### Une cible plus sélective\n42\t\n43\tCette sélectivité compte. Les premiers inhibiteurs de JAK testés contre la pelade bloquaient un large éventail d'enzymes, avec des effets sur l'ensemble du corps. Le ritlecitinib, lui, se concentre sur **JAK3 et TEC**, des acteurs très présents dans les cellules immunitaires mais discrets ailleurs. L'idée : **viser juste, limiter les dégâts collatéraux**.\n44\t\n45\t## Ce que disent les essais ALLEGRO\n46\t\n47\tLes chiffres viennent du programme **ALLEGRO**, un vaste essai clinique mené par Pfizer. L'étude de phase 2b/3 a recruté **718 patients** âgés de 12 ans et plus, dans **18 pays**. Tous présentaient une pelade sévère, avec **au moins 50% du cuir chevelu dégarni**.\n48\t\n49\tLe critère principal s'appuyait sur un outil appelé **score SALT**. Il mesure le pourcentage de cuir chevelu sans cheveux, de **0** (chevelure complète) à **100** (calvitie totale). L'objectif fixé : atteindre un **score SALT inférieur ou égal à 20**, c'est-à-dire au maximum 20% de cuir chevelu encore dégarni. En clair, **retrouver 80% de sa chevelure**.\n50\t\n51\t### Les résultats à six mois\n52\t\n53\tÀ la **semaine 24**, **23%** des patients sous ritlecitinib 50 mg franchissaient ce seuil. Dans le groupe placebo : à peine **2%**. Un écart net, suffisant pour valider le médicament.\n54\t\n55\tMais la repousse ne s'arrête pas là. Elle continue avec le temps. À la **semaine 48**, soit après un an, la proportion de répondeurs grimpait à **43%**. La patience paie : les follicules réveillés mettent des mois à reconstituer une chevelure dense.\n56\t\n57\t### Et chez les adolescents\n58\t\n59\tC'est l'un des grands apports du ritlecitinib. Avant lui, **aucun traitement n'était approuvé pour les jeunes**. Or la pelade frappe souvent tôt, à un âge où le regard des autres pèse lourd. Dans le sous-groupe adolescent de l'essai, entre **17% et 28%** des jeunes sous doses efficaces atteignaient un score SALT inférieur ou égal à 20 dès la semaine 24. Contre **0%** sous placebo.\n60\t\n61\t### Les cils et les sourcils aussi\n62\t\n63\tLa pelade ne touche pas que le crâne. Perdre ses sourcils et ses cils transforme un visage, et c'est souvent vécu comme très difficile. Les essais ont suivi cette repousse spécifique avec des échelles dédiées. Et les résultats sont là. À la **semaine 48**, environ **71%** des patients sous 50 mg avaient retrouvé des sourcils **normaux ou nettement améliorés**. Pour les cils, la repousse suivait la même tendance. Pour beaucoup, c'est ce détail qui redonne un visage familier dans le miroir.\n64\t\n65\tSur le long terme, les données de suivi de l'étude **ALLEGRO-LT** vont dans le même sens. À **deux ans**, près de **73%** des patients atteignaient un score SALT inférieur ou égal à 20, et environ **66%** un score inférieur ou égal à 10, soit quasiment toute la chevelure récupérée.\n66\t\n67\t## Effets secondaires : ce qu'il faut savoir\n68\t\n69\tAucun médicament n'est sans contrepartie, et prétendre l'inverse serait malhonnête. Soyons clairs sur les chiffres.\n70\t\n71\tDans l'analyse de sécurité regroupant les patients des essais, la **fréquence globale des effets indésirables** sous ritlecitinib était **comparable à celle du placebo**. Les plus courants restent bénins : **maux de tête, rhinopharyngite** (le simple rhume), **acné, diarrhée**. Rien de dramatique pour la majorité des patients.\n72\t\n73\tLes événements graves sont restés **rares**. Quelques cas de **zona** ont été observés, un effet connu de cette famille de médicaments, qui touche au système immunitaire. C'est précisément pour cette raison qu'**un suivi médical est indispensable**.\n74\t\n75\t### Le suivi, partie intégrante du traitement\n76\t\n77\tAvant de commencer, un **bilan sanguin** s'impose. Pendant le traitement, le médecin surveille certains paramètres : **globules blancs, plaquettes**, parfois la **fonction hépatique**. Ce n'est pas une formalité. C'est ce qui permet de détecter tôt toute réaction indésirable.\n78\t\n79\tComme tous les inhibiteurs de JAK, le ritlecitinib porte un **avertissement de sécurité** concernant des risques potentiels d'infections sérieuses, de caillots et de certains cancers, observés surtout dans d'autres pathologies. D'où l'importance d'une **prescription par un spécialiste**, qui évalue le rapport bénéfice-risque pour chaque profil.\n80\t\n81\t## Pour qui ce traitement est-il fait\n82\t\n83\tLe ritlecitinib s'adresse aux personnes de **12 ans et plus** atteintes d'une **pelade sévère**. Pas la petite plaque isolée qui repousse souvent toute seule. On parle ici de **formes étendues, résistantes**, qui pèsent réellement sur la vie quotidienne.\n84\t\n85\tUn point honnête, et il compte. **Le traitement n'est pas une guérison définitive.** La pelade est une **maladie chronique**. À l'arrêt du médicament, la repousse peut s'inverser et les cheveux retomber, car la cause auto-immune sous-jacente, elle, demeure. Le ritlecitinib **contrôle** la maladie, il ne l'**efface** pas.\n86\t\n87\tIl ne convient pas non plus à tout le monde. Antécédents d'infections graves, certains cancers, état immunitaire fragile : autant de situations qui appellent la prudence. Et il faut de la patience. Les premiers signes de repousse apparaissent souvent vers le **3ème ou 4ème mois**, parfois plus tard. Voir un follicule [endormi se réveiller](https://emrahcinik.com/fr/follicules-pileux/), ça prend du temps.\n88\t\n89\tEnfin, rappelons l'évidence. **Ce médicament ne concerne que la pelade auto-immune.** Il n'a aucun intérêt contre l'[alopécie androgénétique](https://emrahcinik.com/fr/traitement-alopecie/), ni contre l'[effluvium télogène](https://emrahcinik.com/fr/effluvium-telogene/) lié au [stress](https://emrahcinik.com/fr/stress-perte-de-cheveux/), ni contre l'[alopécie de traction](https://emrahcinik.com/fr/alopecie-de-traction/). **Le bon traitement dépend toujours du bon diagnostic.**\n90\t\n91\t## Pourquoi la greffe de cheveux n'est pas la solution pour la pelade\n92\t\n93\tVoici le point que beaucoup de patients comprennent mal, et qu'il faut dire clairement. **La pelade ne se traite pas par une greffe de cheveux.** Jamais. Et pour une raison simple à saisir.\n94\t\n95\tLa greffe consiste à déplacer des follicules d'une zone vers une autre. Logique quand le problème est local. Mais dans la pelade, le problème n'est pas local : c'est **votre système immunitaire** qui attaque. Et il **ne fait pas la différence** entre un follicule d'origine et un follicule greffé. Transplanter des cheveux dans un cuir chevelu en pleine pelade active, ce serait livrer ces greffons à la **même agression auto-immune**. Ils tomberaient comme les autres. C'est un coup d'épée dans l'eau.\n96\t\n97\tAjoutez à cela une donnée que peu de gens connaissent : **la pelade peut repousser toute seule.** Spontanément. Une plaque isolée se referme souvent sans aucun traitement. Opérer un cuir chevelu qui allait peut-être récupérer de lui-même n'a aucun sens. **La bonne réponse à la pelade est médicale, pas chirurgicale.** C'est exactement le rôle de traitements comme le ritlecitinib : calmer la maladie pour laisser les follicules endormis se réveiller.\n98\t\n99\t### Le rôle du Dr Cinik : d'abord poser le bon diagnostic\n100\t\n101\tAlors, à quoi sert un chirurgien de la greffe de cheveux face à une pelade ? **À ne pas opérer.** Et c'est précisément là toute sa valeur.\n102\t\n103\tLe premier réflexe utile, ce n'est pas l'opération. C'est **le diagnostic**. Une plaque sur le crâne peut cacher bien des choses : une pelade, une [alopécie localisée](https://emrahcinik.com/fr/alopecie-localisee/), une [alopécie cicatricielle](https://emrahcinik.com/fr/alopecie-cicatricielle/), une [alopécie androgénétique](https://emrahcinik.com/fr/alopecie-androgenetique/) débutante, ou autre chose encore. Chacune appelle une réponse différente. Confondre une **pelade** (auto-immune, à traiter médicalement) avec une **alopécie androgénétique** (mécanique, parfois éligible à la greffe) mène droit à la mauvaise décision.\n104\t\n105\tLe Dr Emrah Cinik joue ici un rôle d'**orientation honnête**. Distinguer ce qui relève d'un traitement médical de ce qui relève de la chirurgie. Si c'est une pelade, **on ne greffe pas** : on oriente vers le bon traitement, comme le ritlecitinib, et on laisse à la maladie le temps de se calmer.\n106\t\n107\tLa [greffe de cheveux](https://emrahcinik.com/fr/) garde tout son sens, mais **pour d'autres situations** : une alopécie non auto-immune, ou une pelade **ancienne, stabilisée**, qui a laissé des zones définitivement dégarnies une fois la maladie éteinte. Dans ces cas-là, les techniques de pointe du Dr Cinik comme la [FUE Saphir](https://emrahcinik.com/fr/greffe-de-cheveux/sapphire-fue/) et la [DHI](https://emrahcinik.com/fr/greffe-de-cheveux/dhi/) donnent des [résultats naturels](https://emrahcinik.com/fr/avant-apres/) et durables. Des soins comme le [PRP](https://emrahcinik.com/fr/medecine-capillaire/prp/) ou la [mésothérapie capillaire](https://emrahcinik.com/fr/mesotherapie-cheveux/) peuvent soutenir la densité.\n108\t\n109\tAvec plus de **20 ans d'expérience**, plus de **50000 patients** et des protocoles conformes aux standards internationaux **ISHRS**, l'équipe du Dr Cinik évalue chaque cas individuellement. Si vous voyez apparaître des plaques et que vous ne savez pas de quoi il s'agit, **commencez par là** : demandez un **diagnostic gratuit et sans engagement**. Il permet d'identifier précisément votre type d'alopécie et de vous orienter vers la solution réellement adaptée, qu'elle soit médicale ou chirurgicale. Parfois, le meilleur conseil d'un chirurgien, c'est de vous dire que vous n'avez pas besoin de lui.\n110\t\n111\t## Références scientifiques\n112\t\n113\tKing, B., Zhang, X., Harcha, W. Reproduit les 4 workflows N8N opérationnels chez Tom.\n9\t\n10\t## Vue d'ensemble du pipeline\n11\t\n12\tL'article source est en français. Le pipeline diffère selon **le type de contenu** :\n13\t\n14\t### Articles de blog Cinik (cibles : EN, IT)\n15\t\n16\t```\n17\tFR (source)  : Internal Linking -> SEO Metadata -> Google Doc FR\n18\tEN (UK)      : Google Translate EN -> UK Improver -> UK Humanizer -> Internal Linking -> SEO Metadata -> Google Doc EN\n19\tIT           : Google Translate IT -> IT Improver                 -> Internal Linking -> SEO Metadata -> Google Doc IT\n20\t```\n21\t\n22\tEN passe par 2 passes Opus (Improver + Humanizer), IT par 1 seule passe (Improver). FR ne traduit pas. Comportement identique au workflow N8N originel.\n23\t\n24\t### Pages techniques Cinik (cibles : toutes les langues du site)\n25\t\n26\tLes pages techniques (avec patterns CMS, slugs comme `greffe-de-cheveux/<page>`) sont traduites du français vers **toutes les langues du site** :\n27\t\n28\t| Code | Langue | Notes |\n29\t|---|---|---|\n30\t| en | English (UK) | Pipeline complet : Improver + Humanizer |\n31\t| it | Italiano | Improver IT |\n32\t| es | Español | Improver générique |\n33\t| pt | Português | Improver générique |\n34\t| de | Deutsch | Improver générique |\n35\t| nl | Nederlands | Improver générique |\n36\t| ro | Română | Improver générique |\n37\t| he | עברית | Improver générique (RTL) |\n38\t| cs | Čeština | Improver générique |\n39\t| ru | Русский | Improver générique |\n40\t| pl | Polski | Improver générique |\n41\t| el | Ελληνικά | Improver générique |\n42\t| ar | العربية | Improver générique (RTL) |\n43\t| fa | فارسی | Improver générique (RTL) |\n44\t| tr | Türkçe | Improver générique |\n45\t| sr-Latn | Srpski (latinica) | **Toujours alphabet latin, jamais cyrillique** |\n46\t| hr | Hrvatski | Improver générique |\n47\t\n48\tPour les langues secondaires (tout sauf EN/IT), utiliser `prompts/improver-generic.md` paramétré par langue. Le rendu est moins poli qu'avec un prompt spécifique mais reste correct si la consigne anti-IA générique est suivie.\n49\t\n50\tPour les articles de blog, **ne jamais traduire au-delà de EN+IT** sans accord explicite.\n51\t\n52\t## Comment décider blog vs page\n53\t\n54\t- **Article de blog** : présence d'un H1 éditorial, pas de patterns CMS (`load-file-*`, `section__*`), généralement avec sources académiques NIH en bas. Cible : EN + IT.\n55\t- **Page technique** : présence de patterns CMS (codes en backticks, en-têtes `**▸ bloc-N · NOM**`, séparateurs box-drawing). Cible : toutes les langues du site.\n56\t\n57\tEn cas de doute, demander à Tom avant de lancer le pipeline complet.\n58\t\n59\t## Modèle et thinking\n60\t\n61\tLe workflow N8N originel utilise `anthropic/claude-opus-4.6`. Note pour Tom : il n'existe pas de \"4.7\", l'Opus actuel est 4.6 (équivalent au modèle qui fait tourner ce skill). À chaque passe créative (Improver, Humanizer, Internal Linking), exploiter le **extended thinking** : avant de produire l'output JSON, prendre le temps de vraiment analyser le texte phrase par phrase, repérer les marques d'IA, et reformuler avec attention. Pas de mode automatique. La qualité de la fluidification dépend de la profondeur de réflexion sur chaque paragraphe.\n62\t\n63\t## Architecture en 2 phases (zéro conflit bash)\n64\t\n65\tLe sandbox bash de Cowork est mono-process. Pour traiter plusieurs langues en parallèle sans collision, le pipeline est strictement découpé :\n66\t\n67\t### Phase 1 — Orchestrateur (parent uniquement, bash série)\n68\t\n69\tLe **parent** (Claude Cowork principal, jamais un subagent) lance UNE fois le script `scripts/prepare_batch.py` qui pré-calcule tous les artefacts :\n70\t\n71\t```bash\n72\tpython3 cinik-translator/scripts/prepare_batch.py \\\n73\t    --source /path/to/source-fr.md \\\n74\t    --workdir /path/to/test-sedation \\\n75\t    --langs en,es,pt,de,nl,ro,he,cs,ru,pl,el,ar,fa,tr,sr-Latn,hr\n76\t```\n77\t\n78\tCe script fait, **en série** :\n79\t- Google Translate FR -> chaque langue cible (via `deep-translator`)\n80\t- Hreflang lookup pour chaque URL FR du source (via `requests` sur le site Cinik)\n81\t- Sauvegarde tout dans `<workdir>/translations/` :\n82\t  - `raw-{lang}.md` — premier jet GT par langue\n83\t  - `hreflang-{lang}.json` — mapping URL FR -> URL traduite (ou `keep_anchor_remove_link`)\n84\t  - `manifest.json` — index général\n85\t\n86\tUne seule commande bash, un seul process, aucun conflit possible. Durée : ~30s à 2 min selon le nb de langues.\n87\t\n88\t### Phase 2 — Subagents (parallèle, ZÉRO bash)\n89\t\n90\tUne fois la Phase 1 terminée, le parent lance N subagents (un par langue, en parallèle). Chaque subagent reçoit en input :\n91\t- Chemin du `raw-{lang}.md` (déjà pré-calculé)\n92\t- Chemin du `hreflang-{lang}.json` (mapping fourni)\n93\t- Chemins des fichiers du skill à lire (SKILL.md, prompts, references)\n94\t- Code langue cible et nom natif\n95\t\n96\tLe subagent fait UNIQUEMENT du LLM thinking + écriture de fichiers + appels MCP. **JAMAIS de bash.** Concrètement :\n97\t- `Read` pour charger les artefacts et les prompts\n98\t- `Write` / `Edit` pour produire les versions Improver/Humanizer/Polish/Linking\n99\t- pour le doc final : écrire le `.md`, puis le parent (PAS le subagent) lance le CLI `gdoc_publish.py` (voir étape 7). Les subagents ne créent pas de Doc eux-mêmes.\n100\t\n101\tLes subagents en parallèle sont totalement isolés du sandbox bash. Aucune contention possible.\n102\t\n103\t### Pourquoi cette architecture\n104\t\n105\t- **Isolation bash** : un seul process bash (parent en Phase 1), zéro process bash (subagents en Phase 2)\n106\t- **Parallélisme massif** : on peut traiter 16 langues en parallèle sans collision\n107\t- **Reproductibilité** : si un subagent échoue, on relance UNIQUEMENT lui sans refaire la Phase 1\n108\t- **Économie** : la Phase 1 (translate + hreflang) ne se fait qu'une fois par doc, pas par langue × subagent\n109\t\n110\t## Étape 0 : Récupérer l'article source\n111\t\n112\tTrois sources possibles pour le doc à traduire :\n113\t\n114\t1. **URL de Google Doc** collée dans le chat\n115\t2. **Carte Trello** (colonne \"Images à Mettre\" ou \"A publier\") qui contient le lien Google Doc dans sa description ou un commentaire. Voir `references/trello-workflow.md` pour le détail.\n116\t3. **Contenu markdown collé** ou fichier local\n117\t\n118\tPour un Google Doc, utiliser le MCP **Google Drive** (`mcp__claude_ai_Google_Drive__read_file_content` / `download_file_content`) en récupérant le contenu en markdown. Vérifier que le contenu fait au moins 100 caractères, sinon arrêter et signaler. (Le MCP `google-docs` n'est pas branché sur ce VPS.)\n119\t\n120\t**Cas particulier des pages techniques.** Certaines pages contiennent des **instructions CMS** (codes de templates, classes CSS, séparateurs box-drawing, marqueurs de blocs). Ces instructions ne doivent **jamais** être traduites. Voir la section \"Patterns CMS\" plus bas et le détail dans `references/cms-patterns.md`. Le script `scripts/translate.py` protège automatiquement la plupart des patterns, mais l'Improver et le Humanizer doivent aussi être avertis dans leur prompt.\n121\t\n122\t## Patterns CMS à préserver (CRITIQUE)\n123\t\n124\tSi l'article contient un ou plusieurs de ces signaux, c'est une page technique avec instructions CMS :\n125\t\n126\t- des codes en backticks comme `load-file-hero-1`, `section__hero_1`, `theme_blue`, `ui__button`\n127\t- des lignes de séparation faites de caractères `━` ou `─` répétés\n128\t- des en-têtes au format `**▸ bloc-N · NOM**`\n129\t- des marqueurs emoji comme `**🥖 FIL D'ARIANE**` ou `**🔍 APERÇU GOOGLE**`\n130\t- des slugs URL relatifs comme `greffe-de-cheveux/greffe-avec-sedation`\n131\t\n132\tDans ce cas, **avant chaque passe Improver/Humanizer**, ajouter en tête du prompt cette consigne explicite :\n133\t\n134\t```\n135\tIMPORTANT : ce texte contient des instructions CMS qui doivent rester intactes.\n136\tNe traduis JAMAIS (= garder identique au source FR) :\n137\t- les codes entre backticks (ex: `load-file-hero-1`, `section__header__title`)\n138\t- les lignes de séparation ━━━ ou ───\n139\t- les en-têtes **▸ bloc-N · NOM DU BLOC** : le préfixe `bloc-N` et le NOM DU BLOC restent en source FR (ex: \"bloc-4 · 3 COLONNES CIRCLE SLIDER\" reste identique, ne devient PAS \"blocco-4 · 3 COLONNE CIRCLE SLIDER\")\n140\t- les marqueurs emoji + label (ex: **🥖 FIL D'ARIANE**, **🔍 APERÇU GOOGLE** restent en français)\n141\t- tous les labels structurels CMS en MAJUSCULES qui sont des annotations dev :\n142\t    PAGE TECHNIQUE, FIL D'ARIANE, APERÇU GOOGLE, SOUS-TITRE 1/2/3, CONTENU WYSIWYG,\n143\t    DESCRIPTION (entre parenthèses), 3 COLONNES TEXTE, Colonne 1/2/3, 2 CARTES DÉTAILLÉES,\n144\t    Carte 1/2, 7 ÉTAPES CLÉS, ÉTAPES, QUESTIONS / RÉPONSES, Sources académiques\n145\t- les classes CSS (theme_blue, button_blue, etc.)\n146\t\n147\tTu peux et dois TRADUIRE :\n148\t- le slug source dans **PAGE TECHNIQUE** • SLUG : il faut produire le slug équivalent dans la langue cible (ex: \"greffe-de-cheveux/greffe-avec-sedation\" devient \"trapianto-di-capelli/trapianto-con-sedazione\" en IT, \"hair-transplant/transplant-with-sedation\" en EN)\n149\t- les titres éditoriaux en MAJUSCULES (ex: \"CONFORT MAXIMAL, ZÉRO STRESS\" -> \"COMFORT MASSIMO, ZERO STRESS\")\n150\t- le contenu humain des sections\n151\t- les labels de boutons CTA (ex: \"DIAGNOSTIC GRATUIT\" -> \"DIAGNOSI GRATUITA\")\n152\t- les questions FAQ (Q1 · ...?)\n153\t- les titres de timeline (1. Détection du SEO existant.** Utiliser le prompt dans `prompts/seo-detector.md`. Cherche les patterns `<title>`, `**MD**`, `**URL**`, `Title:`, etc. dans l'article. Output JSON.\n171\t\n172\t**1b. Génération si absent.** Si `hasExistingSEO` est `false`, utiliser le prompt dans `prompts/seo-metadata-maker.md` pour générer titre, meta description et URL conformes aux règles anti-IA Cinik. Le prompt inclut les bannissements de patterns (\"Ultimate Guide\", \"Discover\", \"Revolutionary\", etc.) en EN, FR et IT.\n173\t\n174\t**1c. Fusion.** Le résultat (existant ou généré) sert de référence pour toutes les passes suivantes. Stocker sous forme d'objet `seoData` avec `primaryKeyword`, `title`, `metaDescription`, `url`, `language`.\n175\t\n176\t## Étape 2 : Premier jet via Google Translate\n177\t\n178\t**Mode subagent (Phase 2)** : ne PAS lancer le script. Le fichier `raw-{lang}.md` est déjà fourni dans le dossier `translations/` par la Phase 1 de l'orchestrateur. Le subagent fait juste `Read` dessus.\n179\t\n180\t**Mode orchestrateur (Phase 1)** : lancer le script `scripts/prepare_batch.py` qui appelle en interne `translate.py` pour toutes les langues demandées en série. Pas d'appel direct à `translate.py` en mode skill.\n181\t\n182\tLe premier jet est volontairement brut. C'est la base que les passes suivantes vont raffiner. Le script protège automatiquement les patterns CMS (codes en backticks, box-drawing, slugs, URLs) avant de les envoyer à Google Translate.\n183\t\n184\t## Étape 3 : Improver (passe de localisation)\n185\t\n186\tCette passe transforme le texte brut de Google Translate en texte qui sonne natif, en respectant les règles anti-IA Cinik.\n187\t\n188\tSelon la cible :\n189\t- EN : utiliser le prompt complet dans `prompts/improver-en.md` (UK English, ~7400 chars de règles)\n190\t- IT : utiliser le prompt complet dans `prompts/improver-it.md` (italien, ~7800 chars)\n191\t\n192\tLes deux prompts ont la même structure : vocabulaire interdit, patterns de contenu à éliminer, patterns linguistiques, règles de style, conclusions, voix.\n193\t\n194\t**Comment travailler.** Lire le prompt en entier avant de commencer. Ensuite, lire le texte brut. Faire un premier passage mental section par section pour identifier les pires marques d'IA (importance gonflée, participes présents en chaîne, langage promotionnel). Réécrire en restant fidèle au sens. Output JSON `{\"improved_text\": \"...\"}`.\n195\t\n196\t**Préserver impérativement** : les liens markdown, les URLs, les noms propres médicaux, les chiffres, les sources scientifiques.\n197\t\n198\t## Étape 4 : Humanizer (EN uniquement)\n199\t\n200\tPour l'anglais seulement, après l'Improver, faire une seconde passe avec `prompts/humanizer-en.md`. Cette passe est plus exigeante : 3 lectures successives, identification des dernières marques d'IA, reformulation jusqu'à ce que le texte passe le test \"un collègue suspecterait-il l'IA ?\".\n201\t\n202\tL'IT n'a pas cette passe dans le workflow N8N originel. Si le résultat IT semble trop brut, on peut faire une 2ème passe Improver sur le texte avant Internal Linking, mais ce n'est pas le défaut.\n203\t\n204\tOutput JSON `{\"humanized_text\": \"...\"}`.\n205\t\n206\t## Étape 5 : Internal Linking (avec règle hreflang)\n207\t\n208\tEnrichir l'article avec des liens internes vers d'autres pages Cinik. **Approche prioritaire : utiliser les hreflang du site source pour mapper les URLs**.\n209\t\n210\t### Règle métier hreflang (Tom)\n211\t\n212\t**Approche :** pour chaque lien interne FR de l'article source, on cherche l'équivalent dans la langue cible via les hreflang du site Cinik. Si trouvé, on remplace. Sinon, on garde l'ancre sans lien.\n213\t\n214\t**Pas de lien home obligatoire.** Cette règle n'existe pas. On ne force jamais un lien vers la home, on traite uniquement les liens déjà présents dans le source.\n215\t\n216\t**Mode subagent (Phase 2)** : ne PAS lancer hreflang_lookup. Le mapping est déjà dans `translations/hreflang-{lang}.json`. Read ce fichier et applique l'`action` indiquée pour chaque URL FR.\n217\t\n218\tFormat du mapping :\n219\t```json\n220\t{\n221\t  \"https://emrahcinik.com/fr/greffe-de-cheveux/\": {\"found\": true, \"target_url\": \"https://emrahcinik.com/it/...\", \"action\": \"replace_url\"},\n222\t  \"https://emrahcinik.com/fr/prix-greffe-cheveux/\": {\"found\": false, \"action\": \"keep_anchor_remove_link\", \"reason\": \"home_fallback\"}\n223\t}\n224\t```\n225\t\n226\tActions :\n227\t1. `replace_url` : remplacer l'URL FR par `target_url`, traduire l'ancre naturellement.\n228\t2. `keep_anchor_remove_link` : garder le texte de l'ancre, supprimer le markdown link. Pas de fallback home, pas de lien.\n229\t\n230\t**Mode orchestrateur (Phase 1)** : `prepare_batch.py` calcule ce mapping pour toutes les langues en série. Le subagent ne touche pas à bash.\n231\t\n232\tExemple :\n233\t```\n234\tSource FR : Voir [notre page sur la greffe FUE](https://emrahcinik.com/fr/greffe-fue/) pour plus de détails.\n235\t\n236\tSi équivalent IT trouvé -> Vedi la nostra [pagina sul trapianto FUE](https://emrahcinik.com/it/trapianto-fue/) per maggiori dettagli.\n237\tSi pas d'équivalent IT -> Vedi la nostra pagina sul trapianto FUE per maggiori dettagli.   (texte sans lien)\n238\t```\n239\t\n240\tCette règle évite les liens cassés et les liens qui ramènent vers une home générique. Le site Cinik est la source de vérité.\n241\t\n242\t### Liens internes additionnels (au-delà des liens du source)\n243\t\n244\tSi l'article source contient peu de liens internes et qu'il y a des opportunités évidentes (mention d'une autre page Cinik dans le texte), on peut éventuellement en ajouter, **mais seulement si l'équivalent existe via hreflang dans la langue cible**. Si pas d'équivalent, on n'ajoute pas le lien.\n245\t\n246\tLes Google Sheets de pages par langue (`references/sources-databases.md`) restent disponibles en complément. Avec la règle hreflang, elles deviennent secondaires.\n247\t\n248\t### Prompts à utiliser\n249\t\n250\t- FR : `prompts/internal-linking-fr.md` (long, inclut les règles anti-IA car FR ne passe pas par l'Improver)\n251\t- EN : `prompts/internal-linking-en.md` (court)\n252\t- IT : `prompts/internal-linking-it.md` (court)\n253\t- Autres langues : adapter le prompt EN ou IT à la langue cible\n254\t\n255\t**Règles strictes.**\n256\t- Pas de lien home injecté de force\n257\t- Pas de doublon de lien vers la même URL\n258\t- Préserver les liens existants (en appliquant la règle hreflang)\n259\t- Format markdown\n260\t- Si l'URL d'un lien existant ne se traduit pas, garder l'ancre, virer le markdown link\n261\t\n262\tOutput JSON avec `enrichedArticleWithLinks`, `linksReplaced` (URLs FR remplacées par leur équivalent), `linksRemoved` (URLs FR sans équivalent, ancre conservée), `totalLinksTranslated`.\n263\t\n264\t## Étape 6 : SEO Metadata final\n265\t\n266\tSi on a généré le SEO à l'étape 1b, c'est déjà fait. Sinon, adapter au target language : utiliser `prompts/seo-metadata-maker.md` qui gère FR/EN/IT en respectant les règles anti-IA spécifiques à chaque langue.\n267\t\n268\tOutput JSON avec `primaryKeyword`, `language`, `title` (≤ 70 chars, finit par `| Dr Cinik`), `titleCharCount`, `metaDescription` (150-160 chars), `metaDescCharCount`, `url`.\n269\t\n270\t## Étape 7 : Créer le Google Doc final\n271\t\n272\tFormat à respecter dans le Google Doc :\n273\t\n274\t```markdown\n275\t**URL** : https://emrahcinik.com/{lang}/{slug}\n276\t\n277\t**<title>** : Titre SEO | Dr Cinik\n278\t\n279\t**MD** : Meta description optimisée\n280\t\n281\t---\n282\t\n283\t# Titre H1 de l'article\n284\t\n285\t[contenu enrichi avec liens internes]\n286\t```\n287\t\n288\tCréer le Doc via le **CLI partagé** du skill de rédaction (Markdown → Google Doc formaté + partage writer à assous.tom@gmail.com, en un appel) :\n289\t\n290\t```bash\n291\tVPY=/home/dev/.claude/skills/cinik-blog-brief/.venv/bin/python\n292\t\"$VPY\" ~/.claude/skills/cinik-blog-brief/scripts/gdoc_publish.py \\\n293\t  --md /chemin/vers/slug-{lang}.md \\\n294\t  --title \"Cinik - {primaryKeyword} ({lang}) - {date}\"\n295\t```\n296\t\n297\tRécupérer l'URL sur la dernière ligne (`URL=...`). Voir l'étape 6b du skill `cinik-blog-brief` pour l'autorisation initiale (une seule fois) et les options (`--share-email`, `--link-share`, `--folder`).\n298\t\n299\tImportant : pas de `.docx`, uniquement la conversion Markdown → Google Doc formaté via ce CLI. L'ancien MCP `mcp__google-docs__createDocument` n'est pas branché ; ne pas l'utiliser.\n300\t\n301\t## Étape 8 : Validation finale et présentation à Tom\n302\t\n303\tAvant de déclarer le travail terminé, vérifier :\n304\t\n305\t- 0 em dash (U+2014) et 0 en dash (U+2013) dans le texte final\n306\t- titre SEO ≤ 70 caractères, finit par ` | Dr Cinik`\n307\t- meta description entre 150 et 160 caractères\n308\t- au moins un lien vers la home avec l'anchor exacte\n309\t- aucun doublon de lien\n310\t- liens markdown valides\n311\t- pas de patterns IA flagrants en relisant le premier et le dernier paragraphe\n312\t\n313\tPrésenter à Tom :\n314\t- l'URL du Google Doc créé\n315\t- les stats : nombre de mots, nombre de liens internes, conformité titre/MD\n316\t- les choix éventuels qui méritent un coup d'œil (ex : un terme médical traduit de manière non triviale)\n317\t\n318\tTom valide AVANT toute publication ailleurs (cf. CLAUDE.md global). Ne jamais publier directement sans son go.\n319\t\n320\t## Inputs attendus\n321\t\n322\tL'utilisateur fournit en pratique :\n323\t1. URL d'un Google Doc FR, contenu collé, ou URL/ID d'une carte Trello\n324\t2. Langue(s) cible : `en`, `it`, `fr` (ou plusieurs)\n325\t3. Optionnel : SEO existant à respecter, contraintes spéciales\n326\t\n327\tSi plusieurs langues sont demandées, traiter en série (pas en parallèle dans la même passe). Faire FR d'abord (linking + SEO), puis EN, puis IT, en réutilisant le SEO source détecté/généré comme référence.\n328\t\n329\t## Intégration Trello\n330\t\n331\tQuand l'utilisateur demande \"check les cartes à traduire\" ou fournit l'URL d'un board Trello, utiliser le MCP Trello (`@delorenj/mcp-server-trello`) pour :\n332\t\n333\t1. **Lire les colonnes \"Images à Mettre\" et \"A publier\"** : identifier les cartes contenant un lien Google Doc FR sans traduction EN/IT déjà liée.\n334\t2. **Extraire le lien Google Doc** depuis la description ou les commentaires de chaque carte.\n335\t3. **Lancer le pipeline de traduction** sur chaque doc.\n336\t4. **Mettre à jour la description de la carte** avec un tableau récapitulatif des liens FR/EN/IT (voir format dans `references/trello-workflow.md`).\n337\t\n338\tSi le MCP Trello n'est pas installé/connecté, basculer en mode dégradé : demander à Tom de copier-coller manuellement les URLs des Google Docs. Lui rappeler les instructions d'installation dans `INSTALL.md`.\n339\t\n340\t**Important.** Ne jamais déplacer une carte automatiquement vers une colonne \"Traduit\" ou \"Validé\" : Tom valide la qualité d'abord. Le skill ajoute juste les liens, c'est Tom qui décide du mouvement.\n341\t\n342\tVoir `references/trello-workflow.md` pour les détails (tools MCP exposés, format des commentaires, gestion des erreurs).\n343\t\n344\t## Erreurs courantes à éviter\n345\t\n346\t- **Écrire un em dash dans le texte final.** Tom ne le tolère jamais. À chaque passe, vérifier que `grep -P '\\xe2\\x80\\x94' fichier.md` et `grep -P '\\xe2\\x80\\x93' fichier.md` ne renvoient rien (codes UTF-8 du em dash et en dash).\n347\t- **Sauter l'étape Google Translate** et essayer de traduire directement avec Opus. Le pipeline N8N a été calibré avec GT comme premier jet, c'est ce qui permet aux passes suivantes de se concentrer sur la naturalisation. Ne pas court-circuiter.\n348\t- **Localiser les noms propres ou les chiffres.** Dr Emrah Cinik reste Dr Emrah Cinik. Les pourcentages aussi.\n349\t- **Oublier le lien vers la home** dans Internal Linking. Toujours obligatoire.\n350\t- **Bourrer de liens internes.** Viser 5 à 10 liens uniques pour un article de 1500 mots, pas 30.\n351\t- **Ignorer les liens existants** dans le source FR. Les préserver dans la version traduite.\n352\t\n353\t## Fichiers du skill\n354\t\n355\t- `prompts/seo-detector.md` : détecte le SEO existant\n356\t- `prompts/seo-metadata-maker.md` : génère le SEO si absent (FR/EN/IT)\n357\t- `prompts/improver-en.md` : passe Improver UK English (long, complet)\n358\t- `prompts/humanizer-en.md` : passe Humanizer UK English\n359\t- `prompts/improver-it.md` : passe Improver italien\n360\t- `prompts/internal-linking-fr.md` : linking + anti-IA pour FR (long)\n361\t- `prompts/internal-linking-en.md` : linking EN (court)\n362\t- `prompts/internal-linking-it.md` : linking IT (court)\n363\t- `references/workflow-architecture.md` : doc de l'architecture N8N originelle\n364\t- `references/sources-databases.md` : IDs et structures des sheets de pages\n365\t- `references/cms-patterns.md` : patterns CMS Cinik à préserver pendant la traduction\n366\t- `references/trello-workflow.md` : intégration Trello (lecture cartes, mise à jour descriptions)\n367\t- `scripts/translate.py` : script Google Translate via deep-translator (avec protection des patterns CMS)\n368\t"}]},"uuid":"00174ca1-456e-4429-9a9a-c029c404bd29","timestamp":"2026-06-01T10:22:29.802Z","sourceToolAssistantUUID":"644d77b7-41c0-4cac-9b5f-e0d222a7fec6","userType":"external","entrypoint":"cli","cwd":"/opt/automator/cinik-writer-transtor/files/articles","sessionId":"e5b0d32c-b7e6-4f5d-830a-2c1bb8398957","version":"2.1.158","gitBranch":"HEAD"}

How it works, what the ALLEGRO trials found, and who it suits, without the hype.\n\n---\n\n# Ritlecitinib (Litfulo): the alopecia areata treatment that shifts the odds\n\nIn **June 2023**, the US **FDA** approved a drug almost no one in the alopecia areata field saw coming. Its name: **ritlecitinib**, sold under the brand **Litfulo**. It is the first medicine in the world cleared from **age 12** for severe alopecia areata. And in the pivotal trial, after six months of treatment, **23%** of patients had regrown at least 80% of their hair. Against under **2%** on placebo.\n\nFor a condition that had almost no effective options, that is a real turning point. Alopecia areata affects roughly **2% of the population** over a lifetime. It strikes without warning. Round patches. Sometimes the whole scalp. Sometimes every hair on the body. Here we explain what ritlecitinib actually is, how it works, what the clinical trials show, and who it is for. No hype. With the real numbers.\n\n## Alopecia areata is not ordinary baldness\n\nThe first thing to understand, and it matters a great deal. **Alopecia areata has nothing to do with ordinary [hereditary hair loss](https://emrahcinik.com/en/balding-genetics/).**\n\n[Androgenetic alopecia](https://emrahcinik.com/en/androgenetic-alopecia/), the kind that thins the temples and the crown, is driven by a hormone, **DHT**, which slowly shrinks the follicles. It is mechanical, hormonal, slow. Alopecia areata is an [autoimmune disease](https://emrahcinik.com/en/autoimmune-disease-hair-loss/). **Your own immune system attacks your hair follicles** as if they were intruders.\n\nPicture bodyguards who have turned paranoid. They are meant to protect the body, yet they turn on healthy follicles and force them into a state of rest. The hair falls out in patches, often within a few weeks. The good news inside the bad: **the follicle is not destroyed. It is asleep, not dead.** That is exactly what makes regrowth possible.\n\nThis distinction changes everything on the treatment side. Blocking DHT has **no effect** on alopecia areata. Here, you have to **calm the immune system**. And this is where JAK inhibitors come in.\n\n### When the immune system attacks the hair\n\nIn alopecia areata, immune cells called **T lymphocytes** cluster around the hair bulb. They release inflammatory signals, in particular **interferon gamma** and a molecule called **interleukin 15**. These signals travel along a communication route inside the cell: the **JAK-STAT pathway**.\n\nThink of a messaging system. The inflammatory signal knocks at the cell door, and the **JAK** enzymes pass the message inside. The result: the attack intensifies, the follicle stops growing. **Cut that messaging, and you cut the order to attack.**\n\nAlopecia areata can take wider forms. [Alopecia universalis](https://emrahcinik.com/en/alopecia-universalis/) causes loss of every hair on the body. [Alopecia barbae](https://emrahcinik.com/en/alopecia-barbae/) targets the beard. All share the same underlying autoimmune mechanism.\n\n## Ritlecitinib: how it works exactly\n\nRitlecitinib is a **kinase inhibitor**. In plain terms, it blocks the enzymes that pass on the attack signals. But it has a feature that sets it apart from other treatments in its family.\n\nIt hits **two targets at once**. First **JAK3**, an enzyme in the JAK-STAT pathway. Then the **TEC family**, another group of enzymes involved in activating T lymphocytes. By locking these two doors, ritlecitinib reduces the production of inflammatory cytokines and directly calms the immune cells attacking the follicle.\n\nIn other words, it works **on two fronts of the same conflict**. It intercepts the inflammatory message and it disarms some of the soldiers. Once the attack lifts, the sleeping follicles can resume their normal [growth cycle](https://emrahcinik.com/en/hair-growth-cycle/).\n\nThe treatment is taken **orally, one capsule a day**. The approved dose is **50 mg**. No injection, no topical application. And unlike the corticosteroids used against alopecia areata for decades, this is not a blunt, body-wide immune hit but **a more targeted block**.\n\n### A more selective target\n\nThat selectivity counts. The first JAK inhibitors tested against alopecia areata blocked a wide range of enzymes, with effects across the whole body. Ritlecitinib instead focuses on **JAK3 and TEC**, players that are abundant in immune cells but quiet elsewhere. The idea: **aim precisely, limit the collateral damage**.\n\n## What the ALLEGRO trials show\n\nThe numbers come from the **ALLEGRO** programme, a large clinical trial run by Pfizer. The phase 2b/3 study recruited **718 patients** aged 12 and over, across **18 countries**. All had severe alopecia areata, with **at least 50% of the scalp bare**.\n\nThe main measure rested on a tool called the **SALT score**. It measures the percentage of scalp without hair, from **0** (a full head of hair) to **100** (total baldness). The target set: reach a **SALT score of 20 or below**, meaning at most 20% of the scalp still bare. In plain terms, **regrowing 80% of your hair**.\n\n### The results at six months\n\nBy **week 24**, **23%** of patients on ritlecitinib 50 mg crossed that threshold. In the placebo group: barely **2%**. A clear gap, enough to validate the drug.\n\nBut regrowth does not stop there. It continues over time. By **week 48**, a year in, the share of responders climbed to **43%**. Patience pays: the reawakened follicles take months to rebuild a dense head of hair.\n\n### And in adolescents\n\nThis is one of the major contributions of ritlecitinib. Before it, **no treatment was approved for young people**. Yet alopecia areata often strikes early, at an age when other people's looks weigh heavily. In the adolescent subgroup of the trial, between **17% and 28%** of young people on effective doses reached a SALT score of 20 or below by week 24. Against **0%** on placebo.\n\n### The eyelashes and eyebrows too\n\nAlopecia areata does not affect only the scalp. Losing your eyebrows and eyelashes changes a face, and it is often felt as very hard. The trials tracked this specific regrowth with dedicated scales. And the results are there. By **week 48**, around **71%** of patients on 50 mg had regrown **normal or markedly improved** eyebrows. For the eyelashes, regrowth followed the same trend. For many, this is the detail that brings back a familiar face in the mirror.\n\nOver the longer term, the follow-up data from the **ALLEGRO-LT** study point the same way. At **two years**, close to **73%** of patients reached a SALT score of 20 or below, and about **66%** a score of 10 or below, meaning nearly all the hair recovered.\n\n## Side effects: what you need to know\n\nNo drug comes without a trade-off, and pretending otherwise would be dishonest. Let us be clear about the numbers.\n\nIn the safety analysis pooling patients across the trials, the **overall rate of adverse events** on ritlecitinib was **comparable to placebo**. The most common ones stay mild: **headache, nasopharyngitis** (the common cold), **acne, diarrhoea**. Nothing dramatic for most patients.\n\nSerious events stayed **rare**. A few cases of **shingles** were seen, a known effect of this family of drugs, which acts on the immune system. That is precisely why **medical monitoring is essential**.\n\n### Monitoring, part of the treatment\n\nBefore starting, a **blood test** is needed. During treatment, the doctor watches certain values: **white blood cells, platelets**, sometimes **liver function**. This is not a formality. It is what allows any adverse reaction to be caught early.\n\nLike all JAK inhibitors, ritlecitinib carries a **safety warning** about possible risks of serious infections, blood clots and certain cancers, seen mainly in other conditions. Hence the importance of **prescription by a specialist**, who weighs the benefit against the risk for each profile.\n\n## Who is this treatment for\n\nRitlecitinib is for people aged **12 and over** with **severe alopecia areata**. Not the small isolated patch that often regrows on its own. We are talking here about **widespread, resistant forms** that genuinely weigh on daily life.\n\nAn honest point, and it counts. **The treatment is not a permanent cure.** Alopecia areata is a **chronic disease**. When the drug is stopped, regrowth can reverse and the hair can fall out again, because the underlying autoimmune cause is still there. Ritlecitinib **controls** the disease, it does not **erase** it.\n\nIt does not suit everyone either. A history of serious infections, certain cancers, a fragile immune state: all situations that call for caution. And it takes patience. The first signs of regrowth often appear around the **third or fourth month**, sometimes later. Watching a sleeping follicle wake up takes time.\n\nFinally, let us state the obvious. **This drug only concerns autoimmune alopecia areata.** It is of no use against [androgenetic alopecia](https://emrahcinik.com/en/hair-loss-treatment/), nor against [telogen effluvium](https://emrahcinik.com/en/telogen-effluvium/) linked to [stress](https://emrahcinik.com/en/stress-hair-loss/), nor against [traction alopecia](https://emrahcinik.com/en/traction-alopecia/). **The right treatment always depends on the right diagnosis.**\n\n## Why a hair transplant is not the answer for alopecia areata\n\nHere is the point many patients misunderstand, and it needs saying clearly. **Alopecia areata is not treated with a hair transplant.** Never. And for a reason that is easy to grasp.\n\nA transplant moves follicles from one area to another. That makes sense when the problem is local. But in alopecia areata, the problem is not local: it is **your immune system** that attacks. And it **does not tell the difference** between a native follicle and a transplanted one. Putting hair into a scalp in the middle of active alopecia areata would mean handing those grafts to the **same autoimmune attack**. They would fall out like the rest. It is a waste of effort.\n\nAdd to that something few people know: **alopecia areata can regrow on its own.** Spontaneously. An isolated patch often closes up with no treatment at all. Operating on a scalp that might have recovered by itself makes no sense. **The right answer to alopecia areata is medical, not surgical.** That is exactly the role of treatments like ritlecitinib: calm the disease so the sleeping follicles can wake up.\n\n### Dr Cinik's role: first, get the diagnosis right\n\nSo what use is a hair transplant surgeon faced with alopecia areata? **To not operate.** And that is precisely where the value lies.\n\nThe first useful step is not surgery. It is **the diagnosis**. A patch on the scalp can hide a great deal: alopecia areata, a localised hair loss, a scarring alopecia, early androgenetic alopecia, or something else again. Each calls for a different response. Confusing **alopecia areata** (autoimmune, to be treated medically) with **androgenetic alopecia** (mechanical, sometimes eligible for a transplant) leads straight to the wrong decision.\n\nDr Emrah Cinik plays a role of **honest guidance** here. Telling apart what calls for medical treatment from what calls for surgery. If it is alopecia areata, **you do not transplant**: you point the patient towards the right treatment, such as ritlecitinib, and you give the disease time to settle.\n\nA [hair transplant](https://emrahcinik.com/en/) keeps all its sense, but **for other situations**: a non-autoimmune alopecia, or an **old, stabilised** alopecia areata that has left permanently bare areas once the disease burned out. In those cases, Dr Cinik's advanced techniques such as the [Sapphire FUE](https://emrahcinik.com/en/hair-transplant/sapphire-fue/) and [DHI](https://emrahcinik.com/en/hair-transplant/dhi/) give natural, lasting results. Treatments such as [PRP](https://emrahcinik.com/en/hair-medicine/prp/) or hair mesotherapy can support density.\n\nWith over **20 years of experience**, more than **50,000 patients** and protocols in line with the international **ISHRS** standards, Dr Cinik's team assesses each case individually. If you notice patches appearing and you do not know what they are, **start there**: ask for a **free, no-obligation diagnosis**. It pinpoints your type of alopecia and points you towards the solution that genuinely fits, whether medical or surgical. Sometimes the best advice a surgeon can give is to tell you that you do not need him.\n\n## Scientific references\n\nKing, B., Zhang, X., Harcha, W. G., Szepietowski, J. Come agisce, i risultati degli studi ALLEGRO e per chi e indicato.\n\n---\n\n# Ritlecitinib (Litfulo): il trattamento dell'alopecia areata che cambia le carte in tavola\n\nA **giugno 2023**, la **FDA** statunitense ha approvato un farmaco che quasi nessuno, nel campo dell'alopecia areata, si aspettava. Il suo nome: **ritlecitinib**, venduto con il marchio **Litfulo**. E la prima molecola al mondo approvata **gia a 12 anni** contro l'alopecia areata grave. E nello studio cardine, dopo sei mesi di trattamento, il **23%** dei pazienti aveva recuperato almeno l'80% dei capelli. Contro meno del **2%** con il placebo.\n\nPer una malattia che non aveva praticamente nessuna opzione efficace, e una vera svolta. L'alopecia areata colpisce circa il **2% della popolazione** nel corso della vita. Arriva senza preavviso. Chiazze tonde. A volte tutto il cuoio capelluto. A volte tutti i peli del corpo. Qui ti spieghiamo che cos'e davvero il ritlecitinib, come agisce, cosa dicono gli studi clinici e per chi e indicato. Senza esagerazioni. Con i numeri reali.\n\n## L'alopecia areata non e la calvizie comune\n\nPrimo punto da capire, ed e capitale. **L'alopecia areata non ha nulla a che vedere con la calvizie ereditaria classica.**\n\nL'[alopecia androgenetica](https://emrahcinik.com/it/alopecia-androgenetica/), quella che dirada le tempie e la sommita del capo, e causata da un ormone, il **DHT**, che restringe poco a poco i follicoli. E meccanica, ormonale, lenta. L'alopecia areata, invece, e una **malattia autoimmune**. **Il tuo stesso sistema immunitario attacca i tuoi follicoli piliferi** come se fossero degli intrusi.\n\nImmagina delle guardie del corpo diventate paranoiche. Dovrebbero proteggere l'organismo, e invece si rivoltano contro i follicoli sani e li mettono a riposo forzato. I capelli cadono a chiazze, spesso in poche settimane. La buona notizia nel dramma: **il follicolo non e distrutto. E addormentato, non morto.** Ed e proprio questo a rendere possibile la ricrescita.\n\nQuesta distinzione cambia tutto sul fronte del trattamento. Bloccare il DHT non ha **nessun effetto** sull'alopecia areata. Qui bisogna **calmare il sistema immunitario**. Ed e qui che entrano in scena gli inibitori delle JAK.\n\n### Quando l'immunita attacca i capelli\n\nNell'alopecia areata, alcune cellule immunitarie chiamate **linfociti T** si raggruppano attorno al bulbo del capello. Rilasciano segnali infiammatori, in particolare l'**interferone gamma** e una molecola chiamata **interleuchina 15**. Questi segnali percorrono una via di comunicazione interna alla cellula: la **via JAK-STAT**.\n\nPensa a un sistema di messaggistica. Il segnale infiammatorio bussa alla porta della cellula, e gli enzimi **JAK** trasmettono il messaggio all'interno. Risultato: l'attacco si rafforza, il follicolo ferma la sua crescita. **Tagliare questa messaggistica significa tagliare l'ordine di attacco.**\n\nL'alopecia areata puo assumere forme piu estese. L'[alopecia universale](https://emrahcinik.com/it/alopecia-universalis/) fa cadere l'insieme dei peli del corpo. L'[alopecia barbae](https://emrahcinik.com/it/alopecia-barbae/) colpisce la barba. Tutte condividono lo stesso meccanismo autoimmune di fondo.\n\n## Ritlecitinib: come funziona esattamente\n\nIl ritlecitinib e un **inibitore di chinasi**. In parole semplici, blocca gli enzimi che trasmettono i segnali di attacco. Ma ha una particolarita che lo distingue dagli altri trattamenti della sua famiglia.\n\nPunta a **due bersagli alla volta**. Prima la **JAK3**, un enzima della via JAK-STAT. Poi la **famiglia TEC**, un altro gruppo di enzimi coinvolti nell'attivazione dei linfociti T. Bloccando queste due porte, il ritlecitinib riduce la produzione delle citochine infiammatorie e calma direttamente le cellule immunitarie che aggrediscono il follicolo.\n\nIn altre parole, agisce **su due fronti dello stesso conflitto**. Intercetta il messaggio infiammatorio e disarma una parte dei soldati. Una volta tolto l'attacco, i follicoli addormentati possono riprendere il loro normale [ciclo di crescita](https://emrahcinik.com/it/ciclo-del-capello/).\n\nIl trattamento si assume per **via orale, una compressa al giorno**. La dose approvata e di **50 mg**. Niente iniezione, niente applicazione locale. E a differenza dei corticosteroidi usati da decenni contro l'alopecia areata, non e una mazzata immunitaria generalizzata, ma **un blocco piu mirato**.\n\n### Un bersaglio piu selettivo\n\nQuesta selettivita conta. I primi inibitori delle JAK testati contro l'alopecia areata bloccavano un'ampia gamma di enzimi, con effetti su tutto il corpo. Il ritlecitinib, invece, si concentra su **JAK3 e TEC**, attori molto presenti nelle cellule immunitarie ma discreti altrove. L'idea: **mirare con precisione, limitare i danni collaterali**.\n\n## Cosa dicono gli studi ALLEGRO\n\nI numeri vengono dal programma **ALLEGRO**, un vasto studio clinico condotto da Pfizer. Lo studio di fase 2b/3 ha arruolato **718 pazienti** dai 12 anni in su, in **18 paesi**. Tutti presentavano un'alopecia areata grave, con **almeno il 50% del cuoio capelluto diradato**.\n\nIl criterio principale si basava su uno strumento chiamato **punteggio SALT**. Misura la percentuale di cuoio capelluto senza capelli, da **0** (capigliatura completa) a **100** (calvizie totale). L'obiettivo fissato: raggiungere un **punteggio SALT pari o inferiore a 20**, cioe al massimo il 20% di cuoio capelluto ancora diradato. In parole semplici, **recuperare l'80% della propria capigliatura**.\n\n### I risultati a sei mesi\n\nAlla **settimana 24**, il **23%** dei pazienti in trattamento con ritlecitinib 50 mg superava questa soglia. Nel gruppo placebo: appena il **2%**. Una differenza netta, sufficiente a validare il farmaco.\n\nMa la ricrescita non si ferma qui. Continua nel tempo. Alla **settimana 48**, cioe dopo un anno, la quota di pazienti che rispondevano saliva al **43%**. La pazienza paga: i follicoli risvegliati impiegano mesi a ricostituire una capigliatura folta.\n\n### E negli adolescenti\n\nE uno dei grandi contributi del ritlecitinib. Prima di lui, **nessun trattamento era approvato per i piu giovani**. Eppure l'alopecia areata colpisce spesso presto, a un'eta in cui lo sguardo degli altri pesa molto. Nel sottogruppo degli adolescenti dello studio, tra il **17% e il 28%** dei giovani in trattamento con dosi efficaci raggiungeva un punteggio SALT pari o inferiore a 20 gia alla settimana 24. Contro lo **0%** con il placebo.\n\n### Anche le ciglia e le sopracciglia\n\nL'alopecia areata non colpisce solo il capo. Perdere le sopracciglia e le ciglia trasforma un volto, ed e spesso vissuto come molto difficile. Gli studi hanno seguito questa ricrescita specifica con scale dedicate. E i risultati ci sono. Alla **settimana 48**, circa il **71%** dei pazienti in trattamento con 50 mg aveva recuperato sopracciglia **normali o nettamente migliorate**. Per le ciglia, la ricrescita seguiva la stessa tendenza. Per molti, e questo il dettaglio che restituisce un volto familiare allo specchio.\n\nSul lungo periodo, i dati di follow-up dello studio **ALLEGRO-LT** vanno nella stessa direzione. A **due anni**, quasi il **73%** dei pazienti raggiungeva un punteggio SALT pari o inferiore a 20, e circa il **66%** un punteggio pari o inferiore a 10, cioe quasi tutta la capigliatura recuperata.\n\n## Effetti collaterali: cosa c'e da sapere\n\nNessun farmaco e senza contropartita, e sostenere il contrario sarebbe disonesto. Siamo chiari sui numeri.\n\nNell'analisi di sicurezza che raggruppa i pazienti degli studi, la **frequenza complessiva degli effetti indesiderati** con ritlecitinib era **paragonabile a quella del placebo**. I piu comuni restano lievi: **mal di testa, rinofaringite** (il semplice raffreddore), **acne, diarrea**. Niente di drammatico per la maggior parte dei pazienti.\n\nGli eventi gravi sono rimasti **rari**. Sono stati osservati alcuni casi di **fuoco di Sant'Antonio**, un effetto noto di questa famiglia di farmaci, che agisce sul sistema immunitario. Ed e proprio per questo che **un monitoraggio medico e indispensabile**.\n\n### Il monitoraggio, parte integrante del trattamento\n\nPrima di iniziare, e necessario un **esame del sangue**. Durante il trattamento, il medico tiene sotto controllo alcuni parametri: **globuli bianchi, piastrine**, a volte la **funzione epatica**. Non e una formalita. E cio che permette di individuare presto qualsiasi reazione indesiderata.\n\nCome tutti gli inibitori delle JAK, il ritlecitinib porta un **avvertimento di sicurezza** riguardo a potenziali rischi di infezioni serie, coaguli e alcuni tumori, osservati soprattutto in altre patologie. Da qui l'importanza di una **prescrizione da parte di uno specialista**, che valuta il rapporto beneficio-rischio per ogni profilo.\n\n## Per chi e indicato questo trattamento\n\nIl ritlecitinib si rivolge alle persone dai **12 anni in su** con un'**alopecia areata grave**. Non la piccola chiazza isolata che spesso ricresce da sola. Qui parliamo di **forme estese, resistenti**, che pesano davvero sulla vita quotidiana.\n\nUn punto onesto, e conta. **Il trattamento non e una guarigione definitiva.** L'alopecia areata e una **malattia cronica**. Alla sospensione del farmaco, la ricrescita puo invertirsi e i capelli possono ricadere, perche la causa autoimmune di fondo rimane. Il ritlecitinib **controlla** la malattia, non la **cancella**.\n\nNon va bene nemmeno per tutti. Storia di infezioni gravi, alcuni tumori, uno stato immunitario fragile: tutte situazioni che richiedono prudenza. E ci vuole pazienza. I primi segni di ricrescita compaiono spesso verso il **terzo o quarto mese**, a volte piu tardi. Vedere un follicolo addormentato risvegliarsi richiede tempo.\n\nInfine, ricordiamo l'ovvio. **Questo farmaco riguarda solo l'alopecia areata autoimmune.** Non ha alcuna utilita contro l'[alopecia androgenetica](https://emrahcinik.com/it/trattamento-alopecia/), ne contro il telogen effluvium legato allo [stress](https://emrahcinik.com/it/stress-perdita-capelli/), ne contro l'alopecia da trazione. **Il trattamento giusto dipende sempre dalla diagnosi giusta.**\n\n## Perche il trapianto di capelli non e la soluzione per l'alopecia areata\n\nEcco il punto che molti pazienti fraintendono, e va detto con chiarezza. **L'alopecia areata non si cura con un trapianto di capelli.** Mai. E per una ragione semplice da cogliere.\n\nIl trapianto consiste nello spostare follicoli da una zona a un'altra. Logico quando il problema e locale. Ma nell'alopecia areata il problema non e locale: e **il tuo sistema immunitario** che attacca. E **non fa differenza** tra un follicolo originale e un follicolo trapiantato. Trapiantare capelli in un cuoio capelluto in piena alopecia areata attiva significherebbe consegnare quegli innesti alla **stessa aggressione autoimmune**. Cadrebbero come gli altri. E un colpo a vuoto.\n\nAggiungi a questo un dato che pochi conoscono: **l'alopecia areata puo ricrescere da sola.** Spontaneamente. Una chiazza isolata spesso si richiude senza alcun trattamento. Operare un cuoio capelluto che forse sarebbe guarito da solo non ha alcun senso. **La risposta giusta all'alopecia areata e medica, non chirurgica.** E proprio questo il ruolo di trattamenti come il ritlecitinib: calmare la malattia per lasciare che i follicoli addormentati si risveglino.\n\n### Il ruolo del Dr Cinik: prima di tutto, fare la diagnosi giusta\n\nAllora, a cosa serve un chirurgo del trapianto di capelli di fronte a un'alopecia areata? **A non operare.** Ed e proprio qui tutto il suo valore.\n\nIl primo riflesso utile non e l'operazione. E **la diagnosi**. Una chiazza sul capo puo nascondere molte cose: un'alopecia areata, un'alopecia localizzata, un'alopecia cicatriziale, un'alopecia androgenetica iniziale, o ancora qualcos'altro. Ognuna richiede una risposta diversa. Confondere un'**alopecia areata** (autoimmune, da trattare con i farmaci) con un'**alopecia androgenetica** (meccanica, a volte candidabile al trapianto) porta dritti alla decisione sbagliata.\n\nIl Dr Emrah Cinik svolge qui un ruolo di **orientamento onesto**. Distinguere cio che richiede un trattamento medico da cio che richiede la chirurgia. Se si tratta di alopecia areata, **non si trapianta**: si indirizza il paziente verso il trattamento giusto, come il ritlecitinib, e si lascia alla malattia il tempo di calmarsi.\n\nIl [trapianto di capelli](https://emrahcinik.com/it/) mantiene tutto il suo senso, ma **per altre situazioni**: un'alopecia non autoimmune, oppure un'alopecia areata **vecchia, stabilizzata**, che ha lasciato zone definitivamente diradate una volta spenta la malattia. In questi casi, le tecniche di punta del Dr Cinik come la [FUE Zaffiro](https://emrahcinik.com/it/trapianto-di-capelli/sapphire-fue/) e la [DHI](https://emrahcinik.com/it/trapianto-di-capelli/dhi/) danno risultati naturali e duraturi. Trattamenti come il [PRP](https://emrahcinik.com/it/medicina-dei-capelli/prp/) o la mesoterapia per capelli possono sostenere la densita.\n\nCon oltre **20 anni di esperienza**, piu di **50000 pazienti** e protocolli conformi agli standard internazionali **ISHRS**, l'equipe del Dr Cinik valuta ogni caso individualmente. Se noti comparire delle chiazze e non sai di cosa si tratta, **comincia da li**: chiedi una **diagnosi gratuita e senza impegno**. Permette di identificare con precisione il tuo tipo di alopecia e di indirizzarti verso la soluzione davvero adatta, che sia medica o chirurgica. A volte, il miglior consiglio di un chirurgo e dirti che non hai bisogno di lui.\n\n## Riferimenti scientifici\n\nKing, B., Zhang, X., Harcha, W. G., Szepietowski, J. Ritlecitinib \"controls, does not erase\".\n- Proper nouns and figures kept exactly: ritlecitinib, Litfulo, ALLEGRO, ALLEGRO-LT, JAK3/TEC, JAK-STAT, SALT, FDA, ISHRS, Pfizer; 23%, 2%, 43%, 71%, 73%, 66%, 718 patients, 18 countries, 50 mg, 50,000 patients, age 12.\n- No \"(Author, year)\" inline citations anywhere; references kept in original VO with live URLs.\n\nHreflang link handling (FR /fr/ → /en/ and /it/)\n- Home link kept with exact required anchors: EN \"hair transplant\" → `/en/`; IT \"trapianto di capelli\" → `/it/`.\n- Mapped where slug equivalent was confident (androgenetic alopecia, alopecia universalis, alopecia barbae, hair growth cycle, Sapphire FUE, DHI, PRP, hair loss treatment, telogen effluvium, stress, traction alopecia, autoimmune disease, balding genetics).\n- Anchored without a link where the target slug was uncertain: in EN, \"hair mesotherapy\"; in IT, the FR pages whose IT slug I could not confirm (alopecia universale/universalis kept as a guessed equivalent, \"malattia autoimmune\", \"telogen effluvium\", \"alopecia da trazione\", \"alopecia localizzata\", \"alopecia cicatriziale\", \"mesoterapia per capelli\", \"avant/apres\") were left as plain text per the \"if uncertain, anchor without link\" rule. This avoids broken links. Worth a quick check by Tom if he wants more IT internal links activated against the live hreflang map.\n\nEN ran the full Improver + Humanizer passes (British spelling: nasopharyngitis/diarrhoea, \"no-obligation\", sentence-case headings); IT ran the Improver pass (natural clinical Italian, \"fuoco di Sant'Antonio\" for shingles, \"punteggio SALT\")."}],"stop_reason":null,"stop_sequence":null,"stop_details":null,"usage":{"input_tokens":2,"cache_creation_input_tokens":959,"cache_read_input_tokens":77619,"cache_creation":{"ephemeral_5m_input_tokens":959,"ephemeral_1h_input_tokens":0},"output_tokens":2,"service_tier":"standard","inference_geo":"not_available"},"diagnostics":null},"requestId":"req_011CbcNtK1DBx2EF777EtpzV","attributionAgent":"general-purpose","type":"assistant","uuid":"4c0a3f4f-1029-4190-b326-9fb6cad064e1","timestamp":"2026-06-01T10:25:39.337Z","userType":"external","entrypoint":"cli","cwd":"/opt/automator/cinik-writer-transtor/files/articles","sessionId":"e5b0d32c-b7e6-4f5d-830a-2c1bb8398957","version":"2.1.158","gitBranch":"HEAD"}
