Il volo in se non danneggia il trapianto. Cio che conta davvero sono alcuni gesti semplici a bordo. Ti spieghiamo prima perche puoi stare tranquillo, poi cosa fare esattamente durante il viaggio.\n14\t\n15\t## Perche puoi volare con tranquillita\n16\t\n17\tCominciamo con lo smontare il mito piu duro a morire: quello della pressione della cabina che \"risucchierebbe\" gli innesti appena impiantati. **E falso.**\n18\t\n19\tGli aerei di linea sono pressurizzati per mantenere un'altitudine cabina equivalente a circa **2400 metri, cioe 8000 piedi**. A questo livello, la saturazione di ossigeno di un passeggero in buona salute scende intorno al **90-93%**, senza il minimo effetto sui follicoli alloggiati nel cuoio capelluto. Nessuna forza meccanica tira gli innesti durante il decollo o l'atterraggio. La depressurizzazione e un falso problema.\n20\t\n21\tPer capire perche gli innesti restano in posizione, bisogna sapere come si ancorano. Subito dopo l'impianto, il corpo produce **fibrina**, una proteina che agisce come una colla biologica: chiude le micro-incisioni e fissa i follicoli. E questo coagulo di fibrina che trattiene tutto nelle prime ore, non una pressione atmosferica perfettamente stabile.\n22\t\n23\t### Dopo quanto tempo gli innesti diventano inestraibili?\n24\t\n25\tLa scienza qui e rassicurante. Uno studio di riferimento condotto su **42 pazienti** ha testato la solidita dell'ancoraggio giorno dopo giorno, semplicemente tirando i capelli innestati. I risultati parlano da soli:\n26\t\n27\t- **Nei primi 2 giorni**, tirare un capello dislocava sistematicamente l'innesto.\n28\t- **Gia al 6 giorno**, l'estrazione diventava molto piu difficile.\n29\t- **Al 9 giorno**, nessun innesto poteva piu essere strappato.\n30\t\n31\tUn solo limite: finche persisteva una crosta aderente, l'innesto restava vulnerabile un po' piu a lungo, fino al 5 giorno. Da qui la regola d'oro che sentirai ovunque: **mai grattare.**\n32\t\n33\tIn altre parole, la finestra davvero fragile e breve. Gli innesti sono ben fissati dopo circa 10 giorni, il tempo che la fibrina lasci il posto a vere connessioni tissutali. Poiche il volo di ritorno cade di solito **entro 1-3 giorni** dall'operazione, il rischio non viene mai dall'aereo in se. Viene da cio che fai, o non fai, all'interno della cabina.\n34\t\n35\t### Cosa si gioca davvero durante la prima settimana\n36\t\n37\tLa grande questione dei primi giorni non e l'altitudine, e l'**ossigenazione**. Nei primi 3 giorni, i follicoli piliferi trapiantati sopravvivono per imbibizione plasmatica: attingono nutrienti e ossigeno direttamente dal liquido dei tessuti circostanti, in mancanza di vasi sanguigni ricollegati. Tra il 2 e il 4 giorno, i vasi esistenti cominciano a collegarsi. Dal 4 al 7 giorno, si formano nuovi vasi che ristabiliscono l'irrigazione completa.\n38\t\n39\tQuesto meccanismo spiega perche la prima settimana pesa cosi tanto sul risultato finale. Un innesto mal ossigenato, disidratato o urtato durante questa fase puo non ricrescere mai. **L'aereo non disturba questo processo.** Ma un cranio che si scalda, che suda e che sfrega contro un poggiatesta, quello si, puo nuocere. Ed e proprio qui che entrano in gioco le tue precauzioni.\n40\t\n41\t## Le precauzioni da prendere durante il volo\n42\t\n43\tOra che abbiamo messo da parte il falso problema, parliamo delle vere fonti di guai. Sono quattro, e la buona notizia e che si gestiscono tutte facilmente.\n44\t\n45\t### 1. Gli sfregamenti, il nemico numero uno\n46\t\n47\tIl pericolo principale e il **contatto diretto sulla zona innestata**. Un poggiatesta su cui lasci cadere la nuca, un cuscino da viaggio mal posizionato, un vicino che ti urta passando, un cappuccio che tiri su per abitudine. Ognuno di questi gesti puo dislocare un innesto nei primi giorni.\n48\t\n49\tLa soluzione e semplice:\n50\t\n51\t- Viaggia con la **testa dritta**, appoggia la schiena piuttosto che il cranio.\n52\t- Usa un **cuscino cervicale a U** che sostiene il collo senza toccare la sommita della testa.\n53\t- Indossa il **cappello post-operatorio** consegnato dalla clinica, largo e non stretto, se ti e stato fornito.\n54\t\n55\tEvita invece di rimettere un berretto comune troppo presto: la questione di portare un berretto dopo un trapianto merita di rispettare i tempi indicati dal tuo chirurgo.\n56\t\n57\t### 2. La sudorazione e il caldo della cabina\n58\t\n59\tUna cabina d'aereo puo diventare in fretta una serra, soprattutto in estate o su un volo affollato. E il sudore, a questo stadio, non e tuo amico: ammorbidisce le croste, alimenta il prurito e aumenta il rischio di piccole infezioni. Gestire la sudorazione dopo un trapianto si riduce a una sola consegna: **resta al fresco.** Regola la bocchetta dell'aria sopra il sedile, mantieni un abbigliamento leggero e traspirante, e resisti alla voglia di grattarti. Le croste che si formano sono normali e cadranno da sole.\n60\t\n61\t### 3. Il sole dal finestrino\n62\t\n63\tSpesso lo si dimentica, ma a **10000 metri di altitudine** la radiazione UV che entra dal finestrino colpisce piu forte che a terra. E un cuoio capelluto appena operato e ultrasensibile: un'esposizione prolungata puo bruciare la pelle e compromettere la cicatrizzazione. Sei vicino al finestrino? **Abbassa la tendina, punto.** Le precauzioni contro il sole dopo un trapianto valgono tanto in aereo quanto su una spiaggia, ed e anche per questo che un trapianto in estate richiede una vigilanza maggiore.\n64\t\n65\t### 4. L'immobilita e la circolazione\n66\t\n67\tQuesto rischio non riguarda i capelli, ma la salute generale. Restare seduti senza muoversi per ore rallenta la circolazione nelle gambe e favorisce la formazione di coaguli, la famosa **trombosi venosa**. Il picco di rischio si colloca nei giorni che seguono un intervento, fino al 10 giorno circa.\n68\t\n69\tUna precisazione che rassicura: un trapianto di capelli e un intervento **leggero**, in anestesia locale, senza allettamento. Il rischio trombotico e molto inferiore a quello di una chirurgia pesante come una protesi d'anca, e i dati suggeriscono che il viaggio aereo non aggiunge un rischio significativo nella maggior parte dei pazienti operati.\n70\t\n71\tPer un volo corto, e trascurabile. Per un lungo raggio, il riflesso da adottare:\n72\t\n73\t- **Alzati ogni due ore** e contrai regolarmente i polpacci.\n74\t- **Idratati bene**: l'aria della cabina e molto secca, spesso sotto il 20% di umidita.\n75\t- **Bevi acqua, non alcol.** Del resto, alcol e trapianto di capelli non vanno mai d'accordo nei giorni che seguono l'operazione.\n76\t\n77\t## Preparare bene il rientro dalla Turchia\n78\t\n79\tLo scenario piu comune tra i nostri pazienti: operazione un giorno, volo di ritorno il giorno dopo o quello successivo. Ecco come affrontare il viaggio con l'animo leggero.\n80\t\n81\tPrima di tutto, **l'abbigliamento**. Prediligi un capo che si infila dal basso o si abbottona, mai un maglione da far passare dalla testa. Eviterai cosi qualsiasi sfregamento, sulla zona donatrice come sulla zona ricevente. Perche preservare la zona donatrice conta quanto proteggere gli innesti: e lei che condiziona un eventuale secondo trapianto in seguito.\n82\t\n83\tPoi, **l'organizzazione a bordo**. Chiedi un posto sul corridoio, per alzarti senza disturbare nessuno e tenere la testa lontana dal finestrino. Tieni con te lo spray fisiologico consegnato dalla clinica, se ti hanno autorizzato a umidificare la zona. Prendi i tuoi antidolorifici all'orario previsto: il trapianto moderno con FUE e quasi indolore, ma un leggero fastidio puo comparire, ed e normale rispetto agli effetti collaterali abituali.\n84\t\n85\tUna parola sul **gonfiore**. Un leggero edema della fronte compare spesso verso il 2 o 3 giorno, a volte proprio durante il viaggio. Niente di grave, scompare da solo in qualche giorno. Dormire e viaggiare in posizione semi-sollevata aiuta a limitarlo, nello stesso spirito dei consigli per dormire bene dopo un trapianto. Al minimo dubbio su un segno anomalo, febbre o dolore insolito che possa far pensare a un'infezione, contatta la tua equipe medica, raggiungibile a distanza.\n86\t\n87\tInfine, sappi cosa aspettarti una volta rientrato. Un leggero prurito con qualche brufolo che va e viene fa parte della **fase di guarigione tardiva**, quando i nuovi follicoli si stanno risvegliando con i loro tempi e il cuoio capelluto sta ancora trovando il suo equilibrio. Comincia a usare un **dermaroller** per riattivare la circolazione e calmare quei piccoli sfoghi, continua a lavare con delicatezza tutti i giorni e, soprattutto, **non grattarti con le unghie**. Ho sospeso semaglutide e testosterone prima dell'intervento. Quando posso riprenderli?\n\nAspetta **circa un mese dall'operazione**. In quel periodo gli innesti si fissano per bene e la prima fase di guarigione si calma. Prima di ricominciare, **senti il medico che te li ha prescritti** e assicurati che il recupero stia procedendo come previsto.\n\n### 3. Mi vengono spesso mal di testa. Posso prendere ibuprofene invece del paracetamolo?\n\nCerto, **l'ibuprofene va benissimo**, e c'e anche un vantaggio in piu: calma pure il gonfiore eventualmente rimasto. Prendilo **a stomaco pieno** per proteggere lo stomaco e resta nella normale dose per adulti.\n\n### 4. Due giorni dopo l'intervento il cuoio capelluto diventa rosso e caldo mentre il resto del corpo e pallido. Devo preoccuparmi?\n\nNiente paura, e proprio quello che ci aspettiamo nei primi giorni. Un cuoio capelluto rosso e caldo significa solo che **il corpo sta mandando piu sangue alla zona per guarirla**. Si calma entro la prima settimana. **Bevi molta acqua** e riposa con la **testa sollevata**.\n\n### 5. Al sesto giorno ho dei piccoli lividi dietro l'orecchio e vicino alla tempia. E un problema?\n\nNo. I piccoli lividi nell'**area donatrice**, dietro l'orecchio o intorno alla tempia, fanno parte della normale guarigione: durante il prelievo dei follicoli sono stati toccati alcuni vasellini. **Spariscono da soli nel giro di una o due settimane**, senza bisogno di alcun trattamento. Di piu rischia di irritare un cuoio capelluto ancora in fase di guarigione. Applicale **con delicatezza, con i polpastrelli**, senza strofinare.\n\n### 9. Dopo quattro mesi ho trovato un linfonodo gonfio dietro la testa, confermato dall'ecografia. Cosa mi consigliate?\n\nUn linfonodo reattivo come questo di solito vuol dire solo che il cuoio capelluto e **un po' irritato in quel punto**. Continua con la tua buona igiene e comincia il **dermaroller a 0,5 mm, tre volte a settimana**, per riattivare la circolazione e far passare l'irritazione. Se dopo **due o tre settimane e ancora gonfio**, torna da noi cosi gli diamo un'occhiata.\n\n### 10. Ho avuto un incidente d'auto sei settimane dopo l'intervento e ora ho una ferita aperta sul cuoio capelluto. Cosa devo fare?\n\nPer prima cosa, **pulisci la ferita con delicatezza usando soluzione fisiologica sterile due volte al giorno** e coprila con una garza sterile non adesiva, mai a mani non lavate. Poi **mettiti subito in contatto con la clinica**, cosi controlliamo che gli innesti siano al sicuro ed escludiamo un'infezione. Una volta che la ferita si e chiusa, un breve ciclo di **[finasteride](https://emrahcinik.com/it/finasteride/) o saw palmetto** aiutera la zona a riprendersi.\n\n### 11. Undici giorni dopo l'intervento il gonfiore e ancora li. E normale?\n\nUn po' di gonfiore puo resistere, ma all'undicesimo giorno vogliamo essere sicuri che **non si stia formando un'infezione**. Comincia per favore con **Augmentin 1 g** come antibiotico di protezione e **mandaci foto nitide**, cosi ti teniamo sotto controllo da vicino. Le croste sono comparse quattro mesi dopo l'intervento e ho avuto la psoriasi in passato. Le due cose possono essere collegate?\n\nPuo darsi. Il tuo cuoio capelluto sembra **molto grasso**, quindi lavalo **a fondo e con regolarita** per togliere gli accumuli. E siccome hai avuto la **psoriasi**, lo stress dell'intervento a volte puo risvegliare la desquamazione in una pelle che ci e predisposta. **Mandaci un breve video**, cosi vediamo quanto e marcata e ti diciamo se uno shampoo medicato puo aiutarti. Posso pulire la zona con qualcosa di diverso dalla soluzione fisiologica sterile?\n\nNei primi giorni, **resta sulla soluzione fisiologica sterile** a meno che non ti abbiamo dato l'ok per qualcos'altro. Molti prodotti contengono **alcol, conservanti o principi attivi** che possono bruciare il cuoio capelluto o mettere a rischio gli innesti. **Mandaci prima il nome esatto** e ti diciamo se e sicuro.\n\n## Si puo fare un trapianto di capelli? Condizioni mediche e idoneita\n\n### 14. Ho una lieve cardiomiopatia, classificata a basso rischio, e sono un atleta attivo. Posso fare un trapianto di capelli?\n\nSi, a patto di mettere in piedi le giuste precauzioni. Dovrai **farti rivedere dal tuo cardiologo poco prima dell'intervento**, con un **ecocardiogramma** e un **via libera scritto** specifico per un trapianto di capelli. Il giorno dell'intervento riduciamo al minimo l'**adrenalina o la evitiamo del tutto**, somministriamo l'anestesia **molto lentamente** e **ti monitoriamo per tutto il tempo** (pressione, frequenza cardiaca, ossigeno, respirazione), idealmente con una **sedazione gestita da un anestesista**. Quindi la risposta e si, ma solo dopo che sia il tuo cardiologo sia l'anestesista ti hanno dato l'ok per iscritto.\n\n### 15. Ho la pressione alta. Un trapianto di capelli e rischioso per me?\n\nSe e **ben controllata, non e un problema**. Ci servono valori recenti e un rapido **via libera del tuo medico**. Una pressione non controllata aumenta il rischio di sanguinamento durante l'intervento, quindi prima la stabilizziamo e poi **la teniamo d'occhio** per tutta la procedura. Continua a prendere i tuoi farmaci come prescritto.\n\n### 16. Ho una tiroide iperattiva e diabete di tipo 2, e prendo allopurinolo, metformina e L-tiroxina. Posso comunque operarmi?\n\nNiente di tutto questo esclude un trapianto. Quello che conta e che entrambe le condizioni siano **stabili**: valori tiroidei nella norma e **diabete ben controllato**, con un'HbA1c costante. Quei farmaci di solito si possono **continuare tranquillamente**. Ci coordiniamo con il tuo **endocrinologo** e potremmo chiederti i risultati recenti della tiroide e della glicemia a digiuno prima di fissarti l'appuntamento.\n\n### 17. Prendo farmaci per la tiroide. Questo mi impedisce di operarmi?\n\nNo, **il farmaco in se non e il problema**. Quello che conta e che la tiroide sia **stabile e ben gestita**. Con i valori ormonali in equilibrio e il via libera del tuo **endocrinologo**, puoi procedere.\n\n### 18. Prendo aspirina. Finche il tuo **cardiologo** ha escluso un problema cardiaco di fondo, un trapianto di capelli e in genere sicuro.
